Activities/Athletics

Eudora Middle School offers a variety of activities and athletic opportunities that allow students to explore interests and strengthen teamwork and confidence. 

All sports and cheer participants must have the following KSHSAA forms on file:

• KSHSAA Physical Form- Must include student and parent/guardian signatures on the front and last page.
• KSHSAA Recommended Concussion & Head Injury Release Form- Required for grades 7-8 and must include student and parent/guardian signatures on the last page.
• Student Care and Safety form (Emergency Medical Treatment)- Completed through PowerSchool.
